潘先生杭州病历翻译

2017年1月11日，市府大楼对面的求是杭州翻译公司*英语翻译赵老师，为潘先生提供了杭州病历翻译。
 
    1月11日下午2:00左右，潘先生来到了求是杭州翻译公司，跟郭老师说，需要为他安排杭州病历翻译。接过潘先生递过来的病历稿子，经扫描后，郭老师便将扫描件发给了公司的*英语翻译赵老师。

    History: more than 2 years ago with no obvious cause, the patient’s scrotum was found a yellow bean size of plague accompanied by itching, without other discomfort pain, so the patient did not attach importance to it, but skin pruritus gradually increase for 2 years. Then he came to our hospital 5 days ago. Pathology examination showed  (20165168 Paget disease outside the breast) now due to skin lesions have existed, in order to seek further treatment, the patient with Pagets disease, admitted to hospital.
 
    Intraoperative see: a red plaque in the size of 8.0cm * 5.0cm of the scrotum , the border is not clear, the skin lesions in the central covered with yellow skin, no obvious ulceration
No oozing exudate, no pus water marks. At around 0 5cm for marking expand, conventional disinfection shop towels with local anesthesia.
 
    Admission record: male patient, 69 years old, due to scrotal red plaques for more than 2 years admitted. Physical examination: the scrotal ministry see a 8cmX 5 cm  size of red patch with unclear boundary, central lesions covered in yellow skin, no rupture, no oozing exudate, no weeping pimple.
 
    Discharge situation: the general condition of the patient was good, no special discomfort. The appearance of the dressing is dry, the incision is well positioned, no oozing of blood oozing fluid, the skin surface is flat, no wave motion, sterile dressing
Medicine when discharge: No
 
    Important findings: (2016-08-10 11:14) electrocardiogram (ECG) examination showed 1 sinus rhythm: 2, left axis deviation, 3 QRS wave group low voltage 3, premature ventricular contractions (2016-08-10; 11:52) CT examination showed: two lungs have a little pulmonary inflammatory lesion, right upper lung lesions with fiber, partial lung nodules slightly small spots, combined with clinical review.
